Implantable defibrillators as originally conceived by Michel Mirowski were limited to the detection and automatic termination of ventricular fibrillation. " The therapeutic intervention was elicited only once deadly polymorphic rhythms had developed. It was rapidly learned, however, that ventricular fibrillation is usually preceded by ventricular tachycardia. Mirowski recognized the pivotal importance of developing algoritllms based on heart rate. Ventricular tachycardia detection allowed the successful development of interventions for the termination of ventricular tachyarrhythmias before they degenerated into ventricular fibrillation. Current device therapy no longer confines itself to tlle termination of chaotic rhythms but seeks to prevent them. Diagnostic algorithms moved upward along the chain of events leading to catastrophic rhytlulls. Rate smoothing algorithms were developed to prevent postextrasystolic pauses from triggering ventricular and atrial tachyarrhytlmlias. Beyond the renaissance of ectopy-centered strategies, long-term prevention received increasing attention. Multisite pacing therapies provided by "Arrhythmia Management Devices" were designed to reduce the "arrhytlunia burden" and optimize the synergy of cardiac contraction and relaxation. Clinical evidence now suggests that atrial fibrillation prevention by pacing is feasible and tllat biventricular pacing may be of benefit in selected patients with heart failure. However, these applications of device therapy that generally require ventricular defibrillation backup remain investigational and were not considered in this book.

About the Author
Bill Colby is the General Counsel for Truman Medical Centers, a multi-hospital safety net health system in Kansas City. He is also the lawyer who represented the family of Nancy Cruzan in their family's right-to-die case, the first such case heard by the U.S. Supreme Court. Bill has appeared on Larry King Live, CNN, NPR, Good Morning America, Frontline, Today, and other national programs, and has spoken across the country on the ethical and legal issues related to death and dying. His second book, Unplugged: Reclaiming Our Right to Die in America (AMACOM 2006), won the 2007 AMWA Medical Book Award Gold Medal in the "Trade" Category. He lives in Prairie Village, Kansas.

Couldn't have been better

A true tragedy that changed the way we look at death...

During my training as a chaplain at Baylor University Medical Center, it was considered part of the "dues" of training that one would take lots of being on-call at the hospital for handling of emergencies. To that end, there was a "call room" where a chaplain could catch a little sleep, while waiting. On one of those sleepless nights in the call room, I viewed a Frontline special on the story of Nancy Beth Cruzan. She was a young woman, fully alive, who, as a result of a terrible accident, would become a test case for end-of-life matters for years to come. After seeing that special, I was deeply touched by the need to convey what our wishes were for the ends of our lives.
The Nancy Beth Cruzan case took the better part of ten years before resolution. The lawyer who fought for her right to be disconnected from the feeding tube was William Colby, the author of this outstanding book. Those of us on the front lines of trying to help families prepare for the issues they will face at the end of life will find insight into the ramifications of that case, as well as grist for the mill of the work that we are doing.
Colby is a highly readable author (at times, I felt like I was reading a Grisham novel), the Cruzan's case is deeply compelling, the story is truly tragic, and readers will come away with an appreciation of the law and concepts that are involved in pursuing these matters. There are several important story lines running throughout this volume: There are the lawyers, one who pulls an unexpected punch; the politicians, aiming for re-election; the Cruzans, especially Nancy's father, Joe, a salt-of-the-earth laborer, broken to the core over the loss of his little girl; a common sense probate judge, just trying to do the right thing; and the right-to-life movement (with whom we generally have sympathy, but not in this case). Indeed, under the skillful telling of Mr. Colby, law itself becomes a character, fickle at times, inflexible at others, and, at the last, compassionate.

Colby is the in-depth and true story of a judicial trial concerning Nancy Cruzan, a woman who was thrown from her vehicle and suffered horrific injuries. Since that tragic accident, Nancy has remained in a coma for five years, until her family abandoned hope for her revival and requested the removal of Nancy's feeding tube so her life could end peacefully. But the state intervened and denied the family's wishes. Thus began a extended legal battle began over who had the authority and the right to authorize the end of medical intervention with respect to a patient like Nancy. Long Goodbye is a profoundly emotional story of striving to do what one hopes is the right thing, in accordance with the wishes of those who cannot speak for themselves -- and the role of government to intrude into family and medical issues. This is a profoundly important issue that plays out in our hospitals and nursing homes every day. At the crux of the matter is the right to life, the right to die, and who has the final authority over a loved one caught up in a plight similar to Nancy Cruzan and her family.

Gotham Central - Read This Now!

It's drawn fantastically by Michael Lark. It's a police procedural. It's set in Gotham City. When factors like these combine to create a comic as incredible as this, it's as if the stars in the heavens above have aligned. Seriously though, Gotham Central is the most unique take on life in Gotham City that I've ever seen, and you'd be remiss in not checking it out.

Brilliant Police Drama in the World of Costumed Vigilantes

Greg Rucka and Ed Brubaker team up to write a stellar police drama set in Batman's home, Gotham City. Michael Lark provides the outstanding art work for this crime drama.

There are three stories contained in this great collection. The first has the writers co-writing a plot introducing us to the night and day shifts of GCPD and the Major Crimes Unit as they take on Mr. Freeze.

Brubaker then tackles the mystery of a kidnapping and murder. With plenty of plots and twists to keep an mystery fan jumping for more.

Rucka takes a more personal look at one of Gotham's Finest, Renee Montoya, as she deals with personal issues and the revenge of a twisted perp she arrested who was released on a technicality.

One of the strongest highlights was seeing how these police officers deal with a city that is protected by the Batman. The spectre of the Batman looms over each story but Batman only makes brief appearances. Brilliant handling by the writers to ground a real world story in the world of costumed vigilantes.

I can't wait for the next volume.

A Great Series in a Beautiful Format

Gotham Central: In the Line of Duty collects issues #1-10 from Greg Rucka and Ed Brubaker's highly acclaimed Batman spin-off title, which had a short-lived run of 40 issues from 2003-2006. The book focuses on Renee Montoya, Crispus Allen and the detectives working GCPD's Major Crimes Unit as they solve cases in a city where Batman's presence is as much a liability as an asset.

If any series ever deserved the hardcover reprint treatment, Gotham Central is it. The book is as good as any police procedural on television, giving names and personalities to the Gotham police force that up until that point had been relegated to minor background characters and cannon fodder for Joker, Two-Face and the like. A tragic run-in with Mr. Freeze in the opening pages is enough to suck readers in, and Rucka and Brubaker continually drive home the cost of being a good cop in a city that relies on Batman for protection. Honestly, Batman's portrayal during this era of comics was never great, often crossing the line from being edgy and gritty into just being unlikable. Gotham Central really puts this in perspective with the cops only asking for assistance from Batman as an absolute last resort (a theme that intensifies in the next volume). Also spotlighted this volume is the relationship between Renee Montoya and Two-Face, which Rucka had been steadily building since Batman: No Man's Land, Vol. 2.

This hardcover edition features the best presentation of Michael Lark's gorgeous artwork, and it's great to have Gotham Central in a format that will include the entire run (as opposed to the earlier trades, which omitted select issues). Two supplemental stores that fill in the Montoya/Two-Face history from the original "Half a Life" trade paperback are not reprinted here - one of them can be found in the aforementioned No Man's Land volume, while the other was also collected in Batman: Evolution (No Man's Land). Readers may also be interested in Batman: Officer Down (New Gotham 2), which shows Gordon's retirement from the force and leads to the status quo presented here.

probably the best of it's kind

While I didn't follow the diet, this is the only book I've found that really has some good combined allergy recipes. There are still a few foods that can cause problems in some of their recipes (eg.tomato, barley) but it still seems like the best I've seen.

This book is EXACTLY the same as "The Elimination Diet Cookbook" by Carter and Edwards. Don't buy them both! The diet is REALLY bizarre. I would recommend one of the other elimination diet books. This one doesn't really make any sense.
